Position Summary

This Behavioral Health Manager- ACT position works as the leader of a multi-disciplinary Assertive Community Treatment team serving transition age youth in the Multnomah County Homeless Youth Continuum.

Essential Duties

  • Participate on treatment team providing Assertive Community Treatment (ACT), Individual Placement and Support (IPS), and Integrated Dual Diagnosis Treatment (IDDT).
  • Support program to comply with all Oregon Administrative Rules related to Assertive Community Treatment (ACT) and follow recommendations of the Oregon Center of Excellence for Assertive Community Treatment (OCEACT). 
  • Support program to comply with all Oregon Administrative Rules related to Individual Placement and Support (IPS) and follow recommendations of the Oregon Supported Employment Center for Excellence (OSECE).  
  • Maintain thorough knowledge of relevant treatment models.
  • Participate in outreach activities necessary to offer services and engage participation in a non-coercive manner.
  • Provide initial comprehensive assessment and re-assessments of participants, including mental status exam, DSM diagnosis, and clinical formulation.
  • Review and sign-off on Assessments and Treatment Plans, discussing clinical formulation as needed.
  • Execute treatment services consistent with professional judgement and licensure as identified on Service Plan, including individual and group counseling and skills interventions.
  • Participate in admission and discharge planning for any participant needing hospitalization or subacute level of care.
  • Provide advocacy and assist participants in accessing the services and other resources they need.
  • Participate in crisis planning and crisis intervention, which may include after-hours work.
  • Participate in the rotation of carrying after-hours crisis phone.
  • Collect treatment outcomes related to behavioral health counseling.
  • Assist in facilitation of team meetings and decision-making.
  • Provide supervision to program staff.
  • Serve as Liaison between RISE and HYC partners.
  • Assist in planning and coordinating program staffing (review of ETO Requests), including staffing plans during peak summer and holiday periods.
  • Support program to comply with all regulatory and funder expectations, including ensuring timely documentation of services and outcomes.
  • Complete administrative tasks directly related to Permanent Supported Housing program compliance.
  • Ensure effective coordination and access to shelter and housing services.
  • Provide holiday coverage, as needed, with other members of the team/program.
  • Adhere to all state and federal privacy regulations, including HIPAA and 42 CFR Part 2, and to Outside In policies and agreements regarding confidentiality, privacy, and security.

Knowledge and Skills

Knowledge

  • Through knowledge of relevant treatment models including Assertive Community Treatment (ACT), Individual Placement and Support (IPS), and Integrated Dual Diagnosis Treatment (IDDT).
  • Knowledge and understanding of adolescent development, Trauma Informed Care, Stages of Change Theory, harm reduction and Motivational Interviewing.

  • Spanish fluency

Qualifications

Education and Experience

  • Oregon LPC or LCSW.
  • Master’s degree in a human services field and meet State guidelines for QMHP.
  • Three years’ experience in mental health counseling.
  • Experience working with homeless or at-risk adolescents.
  • Two years post-licensure experience and able to be qualified as a licensure supervisor preferred.
  • Ability to supervise substance use treatment staff in the state of Oregon including:
    • Chemical Dependency counseling credential (CADC/MAC) or documentation of at least 120 contact hours of academic or continuing professional education in the treatment of substance use disorders.
    • Two years of direct substance use disorders counseling experience.
  • Experience with relevant treatment models including Assertive Community Treatment (ACT), Individual Placement and Support (IPS), and Integrated Dual Diagnosis Treatment (IDDT) preferred.
  • Able to be qualified by Outside In as a Clinical Supervisor  – including demonstrated:
    • Competency in leadership,
    • Competency in wellness,
    • Competency in oversight and evaluation of services,
    • Competency in staff development, and
    • Ability to implement provider policies and procedures.
  • Knowledge of clinical documentation requirements.

Working Conditions

This job includes working in a standard office environment as well as a variety of community locations to offer community-based treatment. This means that the employee will meet with participants in community locations which may include the participant’s home, hospital, and other surrounding community settings. The employee will be responsible for transporting participants in a rented vehicle.

Physical Requirements

  • If in recovery, must be able to document continuous abstinence under independent living conditions or recovery housing for immediate past two years.
  • Must pass pre-employment background check. This includes clearance by the DHS Background Check Unit.
  • Standing, sitting, walking, on a regular basis.
  • Driving, climbing, balancing, reaching, kneeling, crouching, stooping and lifting up to 25 pounds on a frequent basis.
  • Regular use of hands and fingers to type.
  • Talking and hearing is required on a regular basis. 
  • Availability for after hours and holiday coverage rotation.
  • Must be able to climb stairs several times a day.
